Community Care Packages, where services are offered to persons in their own homes, has become an increasingly important focus for aged care providers. Consistent with this, Saint Hilarion has developed an additional business arm SSH Connect as we seek to offer a continuum of care for persons with diverse needs and preferences.
Currently SSH Connect offers Consumer Directed Care (CDC) Home Care packages that provide individualised levels of support to assist older persons to remain living in their own homes. They are operated in conjunction with regulations established by the Commonwealth Government to provide for the complex care needs of older people.

To access a Home Care Package, you will need to be assessed as eligible by an Aged Care Assessment Team, who are usually located at your local hospital. Someone from this team visits your home for the assessment. You will require a referral, which is obtained by calling the My Aged Care – the Federal Government’s aged call centre – on 1800 200 422 or go to www.myagedcare.gov.au.
If you are assessed as eligible for a package, you can ask to have SSH Connect as your preferred service provider. Once you have been offered a package, your provider will help you plan your funding package based on your specific needs and wishes.
