Community care services are available using funding from the Home Care Package. Our community care services are designed to support those to live a full life in their own home. Every package is individually tailored so that it meets identified needs. Our qualified friendly team offer a wide range of services by providing social support and focussing on the types of experiences that many would like to continue to enjoy. There is nothing better than eating fish and chips at the beach, but this proves difficult for those that don’t drive. We can help with this type of activity by providing transport and even sitting and chatting in the fresh sea air.

Interested in Learning Something New?

It is never too late to take up a new hobby or interest. There are lots of places that have interesting classes on everything from computers to woodwork. We help find the desired program and assistance getting to and from a class.

Like Reading but Find it Difficult Because of Poor Eyesight?

There are other ways to enjoy a book if it is difficult to read. Our staff can help with access to library services to choose enjoyable books and to spend time reading out loud on a regular basis. Even a trip to the local library is an enjoyable experience.

Massage is more than a luxury treatment

Physiotherapists and exercise physiologist can provide massage therapy, assisting in better movement as well as helping to treat any physical injuries or health conditions. Massage also helps with circulation, tension and relaxation.

One of our staff can meet with you wherever convenient to plan your care needs and help you to make best use of your funding. We will look through the list of services with you and assist you to choose the services that are right for you. Once you have decided how to spend your funding, the Saint Hilarion Community Care Team will coordinate and manage the services for you.

Assess eligibility for a Home Care Package quickly using the My Aged Care online application. This online application is used for all care and support needs including help at home, short-term care, and care in aged care option options.

How much does it cost?

Home Care Packages are for those with greater or more complex care needs. If ongoing care and support services are needed, eligibility is possible.

Eligibility means the following may be funded:

  • Services that keep a person well and independent
  • Services that keep a person safe in their own home
  • Services that allow a person to interact with their community

The Home Care Package allocation is based on the level of needs. This could range from funding for lawns to be cut once per week to having a carer every day to help prepare for the day and assist with household chores.

There are four levels of Home Care Packages for different care and support needs:

  • Level 1 for basic care needs
  • Level 2 for low-level care needs
  • Level 3 for intermediate care needs
  • Level 4 for high-level care needs.

Each level receives a different amount of government funding. The higher the level, the more funding the government pays, allowing access to more services with Level 4 package allowing purchase of the most services.

Once an Aged Care Assessment is undertaken, a level will be attributed.

Yes. There are a few ways to get the help you need. You can contact My Aged Care through Translating and Interpreting Service (TIS National) on 131 450 for the cost of a local call.

Residential care is where a person moves out of their own home into long-term accommodation. This is usually most suitable for those finding it difficult to continue living independently and safely in their own home.

In-home care is where a person stays living in their own home while receiving support at home to help them continue to remain independent.

Yes definitely. With Saint Hilarion, you can purchase any of our in-home services privately, meaning you cover the cost from your own personal income, rather than with a government subsidy. We will discuss your needs and tailor a Private Care solution to best suit your needs.
